OK if its a kids bed, then I would put a waterproof mattress protector on first, then a fitted sheet (much easier to work with) or for extra comfort a washable blanket between the waterproof cover and the sheet. Until the kids are 14/15 I would always protect the mattress, we all have accidents. Then a duvet 13tog is good, if they do get chilly in the winter just give them a blanket.
For the adults bed I have a cheap, and I mean cheap duvet between the mattress and the mattress cover, then a fitted sheet. A duvet on top. The duvet on the mattress keeps it warm in the winter and cooler in the summer, gives a comfortable base on which to sleep, and if it flattens or gets marked, you can throw it away and get a new one. Most of the big supermarkets do a really cheap range.</description>
